High-Yield Reward Checking Account at an Indiana Bank (Lake City)

Lake City Bank is offering a rewards checking account that pays 1.51% APY on balances up to $50,000 (0.51% for over) and refunds ATM fees (up to $25 per cycle, US only) if the following requirements are met for each statement cycle: 1) 8 POS debit card purchases, 2) one direct deposit or ACH auto debit, and 3) receiving electronic statements. If these are not met, the rate falls to 0.01%. There are no maintenance fees. The minimum opening deposit is $50. Checks are not free.
You can apply at a branch or online, but new customers will need to visit a branch to verify identity. Branches are located in the several Indiana cities, some of which include Fort Wayne, Auburn, Goshen, Elkhart, Middlebury and Warsaw. The bank is FDIC insured (Certificate # 13102)

Update on Lake City Bank, posted 04/01/2010. Their rewards checking is at 3.51% for balances up to $50,000, not $25,000.
