Bank of Blue Valley is implementing a new $5 maintenance fee as of 10/19/11. Can be avoided by signing up for e-statements - this oddly seems to be required even if the account was already on e-statements, because the notice appears on the e-statement.

BEGINNING IN OCTOBER, THE BANK WILL CHARGE A $5.00 MAINTENANCE FEE.
However, you may avoid this monthly fee by arranging to receive your
monthly account statement electronically through our FREE "eStatements"
service, which is a feature of our BlueWave Online Banking.
With eStatements, you will receive an e-mail notification when your
monthly statement is available for viewing. Then, you can quickly and
securely view your online statement, which contains the same information
as your paper statements. You can even print or save a copy if you like.

Their Ultimate Reward Checking pays 3% on $25k but requires $1k in debit card use, and their Performance Reward Checking pays 2% on $15k.
