I have a copy of the advance directive for every member of my family in my "go bag" and provide it to the facility at the time of admission. However, our local hospital (affiliate of a huge national network) requests a copy of the directive, and additionally insists that their own form be completed at every admission. Their form has to be read very carefully as some of the items are worded differently than the advance directive. As a result, one could unwittingly answer in a way that would be contrary to the person's wishes as described in the advance directive. Unfortunately it's an added stress at what is likely an already stressful time.
