Fidelity can't seem to find or locate this form. They only have the form for "mutual funds" not money market funds. Trying to adjust state income taxes- anyone have the form for 2022 or where to find it? Thanks

Unfortunately, you will not find SPRXX, FZDXX, or FMPXX listed in either of these documents because they apparently had no eligible US Government Securities income for 2022. Only money market funds with eligible US Government Securities income above 0.00% are listed. BTW, in 2021 the percentage of eligible income from U.S. government securities for SPRXX and FZDXX was only 3.58%, and FMPXX was only 4.04%, those percentages dropped to 0.00% in 2022. Apparently, Repurchase Agreements of any form (whether U.S. Government, U.S. Treasury, or corporate) are not eligible and are not exempt from state income taxes.
As w00d00w mentioned, you will find FZFXX (Fidelity Treasury Money Market Fund) listed at 29.63% and SPAXX (Fidelity Government Money Market Fund) listed at 30.59% in both of the above documents.
